POLICIES AND INFORMATION

We do not accept online forms or electronically signed forms for services performed by healthcare providers. Forms can be printed and filled out prior to treatment or filled out at the clinic prior to treatment. Without forms signed containing all information we are not able to administer treatment! *PDF of client intakes and direct billing available below

If the client is below 18 years of age, they will require parental or guardian signature at the time of their first appointment! At anytime can a client below 18 years of age have their parent or guardian be present in the room at the time of treatment.

We do not accept direct billing information over the phone or social media. Please remember to bring your physical or virtual card to the appointment - up to a maximum of 3 insurance plans per person can be valid for direct billing. *secondary or tertiary billing is not available to all clients and fully depends on which companies each client has amongst other factors

We do direct bill to almost all insurance companies! A valid alternative form of payment is required to be on file for direct billing. Direct billing payments can take up to two months before the clinic gets paid - at anytime after submission can the insurance company change or cancel those payments. In these cases the client is notified and is more than welcome to pay the owing balance with any form of payment, any information on file is used for no return contact or refusal of payment/fees.

Cancellation Policy: If the client cancels their appointment with less than 24 hours notice there will be a 50% owing fee. At anytime if we are able to fill said appointment the fee will be waived 100%! We provide phone, email and/or text reminders but ultimately it is the clients responsibility for all appointments. *fees will be charged to cards on file if they do not contact the clinic back - they do not qualify for insurance coverage

No Show Policy: If the client does not show up to their appointment, we will provide an extra courtesy call starting at 5 minutes past the appointment start time. You can still come to your appointment even if late! If the client does not respond or contact us back after 15 minutes post treatment start time - there will be a 100% owing fee for said treatment. Please remember we set these time slots aside solely for you - if you are not able to make your appointment somebody else may love that spot. We provide phone, email and/or text reminders but ultimately it is the clients responsibility for all appointments. *fees will be charged to cards on file if they do not contact the clinic back - they do not qualify for insurance coverage

Owing Balances: If the client has not cleared up an owing balance for a cancelled or no show appointment - all future appointments will be cancelled until payment has been received. Clients will not be able to rebook within that owing period.

Right to Refuse: The therapist/healthcare provider has the right to refuse treatment at anytime. We do not tolerate inappropriate or rude comments towards any staff in the clinic.
